Client Profile/Admissions Criteria
- Families with youth involved with Juvenile Probation and/or Children and Youth Services.
- Males and females, ages 8-21 years, referred from Lehigh and Northampton Counties.
Key Services
- Lehigh Valley Community Programs (LVCP) offer an innovative array of community-based programming for delinquent and/or dependent youth for pre-placement intervention, stand-alone treatment and post-placement transitioning.
- All programs are designed to provide intense supervision, direct service and case management.
- Services are delivered in-home, at school and at work, in addition to parent and youth groups on site and a case manager is available 24 hours a day.
- LVCP provides court attendance and testimony for all programs.
- Clients can participate in various activities including baseball games, movies, bowling, etc.
- All per diem services include random drug screens and curfew monitoring at the request of the referring agent.
Programs: Individualized Youth Program (IYP)/Level 1, Youth Services Program (YAP)/Level 2, Supervised Home Services (SHS)/Level 3, Family Preservation Program (FPP)/Level 4, Residential Alternative Program (RAP)/Level 5, and Intensive Family Program (IFP)/Level 6.
New to LVCP is the Reintegration Process which provides the following:
- Case manager's attendance at pre-placement disposition hearings.
- Telephone contact once a week with the youth while in residential care.
- A teleconference with the youth while in residential care to facilitate community treatment planning along with probation officers/case worker and parents/guardians.
- Case managers will visit the home while the youth are in the facility in order to administer a family assessment and initiate treatment planning with the family.
- Contact with the referring agent while youth is in placement to provide updates.
- Case manager will administer a risk assessment to youth and complete treatment plan before the youth returns home.
- Upon return home, youth will enter any one of our treatment programs that will best meet their needs.
Frequently Asked Questions
What is the duration of the programs? The average length of treatments is 6-9 months with minimum weekly face-to-face interactions of 3 for SHS, 4 for FPP, 5 for RAP, and 6 for IFP.
What is the average number of clients per case load? Case managers' caseloads usually consist of 10 to 14 clients for all programs in order to allow case managers the ability to provide intensive services.
What types of support are provided? Parent support groups, youth groups focusing on BARJ principles, AOD, Anger Management, Aggression Replacement, Decision Making, Victim Awareness, Life Skills and Job Readiness.
What is included in Intense Supervision? Individualized treatment based on comprehensive goal planning, drug screening, monitoring and assistance with community service, curfew, restitution collection, school and job attendance.
Which programs are Fee for Service? Individual and Group Counseling.
