Client Profile/Admissions Criteria

  • Male and female offenders from Philadelphia, ages 13 to 21, who are committed to a designated Abraxas residential facility.

  • Case management for court adjudicated youth and their families.

The Abraxas Reintegration Program is committed to assisting clients with their personal growth.  Through this community-based program, we are dedicated to providing reintegration services to adjudicated delinquents and their families in the Philadelphia area.  With a positive role modeling approach, our objective is to promote accountability, competency development and community protection while maintaining respect and dignity to those we serve.  Every effort will be made to link clients with community-based services in order to address educational and social challenges in their respective communities.

Frequently Asked Questions

What is the average length of involvement with the program?
     The length of client involvement with the program will vary depending on the length of time the 
     client is at the facility.  Each client will receive services through the Reintegration Program 
     throughout their stay at the facility, and for 90 days when they return to the community.

Is it mandatory that all clients from Philadelphia who are leaving Abraxas facilities participate in the program?
     Yes.  Any youth who is delinquent and will be residing in Philadelphia County is required to 
     participate in the program.

Do program staff work directly with probation?
     Yes.  Client progress is reported to the assigned probation officer.  One of the program's 
     objectives is to assist clients in adhering to the terms of their probation.

How often will I see my Reintegration Worker?
     While a client is in placement, their Reintegration Worker will visit the client's family in their 
     home at least once a month.  The Reintegration Worker will also visit the client at the facility 
     on a quarterly basis.  Once the client returns to the community, they will see their 
     Reintegration Worker anywhere from one to three times per week depending on the level of 
     need.  

What services will my Reintegration Worker provide?
     Reintegration Workers will act as a liaison between parents, facilities, and probation officers.  
     They will develop a reintegration plan for each client within 60 days of the youth being admitted 
     to the facility.  The reintegration plan will include goals that the youth and their family need to 
     achieve.  The Reintegration Worker will assist the youth and their family in reaching these 
     goals by providing them with the proper resources in their community.
